This survey complements the FLO Orchestra practice-based evaluation of the Hi-Audio online platform. It is divided into three sections and gathers musical and technical background and feedback on your experience using the platform. Most questions are required. For open-ended questions, please feel free to provide any details you consider relevant.
To review session details, participants were directed to open a session-review link in Firefox while signed into their Hi-Audio account.
Consent and Ethical Notice
I agree that Hi-AUDiO may process my data solely for research purposes related to this pilot recording study of the Hi-Audio Online Platform, and confirm my responses are accurate. I may be contacted for clarification if needed.
All data will be treated confidentially and anonymised before any publication or dataset release, and used exclusively for academic research. No personally identifiable information will be disclosed.
Participation is entirely voluntary. I may withdraw at any time without reason or consequence, and request deletion of my data at any point during retention — in which case all associated data will be deleted and excluded from analysis.
Data will be handled under the GDPR and applicable ethical guidelines: securely stored, accessible only to the research team, not shared with third parties, and retained for a maximum of one (1) year before being permanently deleted or anonymised.

Form-builder note (confirmed live 2026-07-03): on the live form this renders as a single unlabeled row (JotForm’s row-name field was left blank — confirmed via the form builder, which shows “Type Row Name” placeholder text, not an actual label; text like “Service Quality” was never part of the form). Functionally this is a single 7-point rating using the question title above as the item wording, not a multi-item matrix. The original JotForm CSV export incorrectly split this single answer into 4 sub-columns suffixed with bogus text (“Service Quality”, “Cleanliness”, “Responsiveness”, “Friendliness”) — a JotForm export defect.
docs/participant_survey_responses.csv/.xlsxhave already been corrected to a single clean column; Q16 is usable as its originally intended headline suitability rating with no further preprocessing.
